Group consultation
Group consultations support you to keep well and help review your condition. You will have a one to one consultation and get to spend more time with your healthcare team in a group setting with fellow patients with similar issues.
To help you get the most from this consultation, some key data relating to your condition may be shared within the group, for example blood pressure, blood test results, etc.
Having other people with the same condition and set of challenges in this session allows you to connected with and support each other.
Patients who receive care in this way tell us they prefer it to shorter and often rushed one to one or telephone appointments.
What will happen in your group consultation appointment
The appointment lasts around 90 minutes.
We will check you in to your appointment and make sure you have completed any pre-session paperwork. Once we have done this you can join the session where our facilitator will greet and guide you through the session.
What to do next
You will need to sign a consent form on the day of the meeting.
We know that group consultations improve health outcomes for people living with multiple conditions. We look forward to seeing you at your group consultation appointment.
