Travel

If you are travelling abroad please ensure you contact us with at least 8 weeks’ notice prior to your travel date to arrange any vaccinations that may be necessary. To help the nurses assess your travel needs it is important that they are in receipt of the travel risk assessment form before your appointment.

Please note, the travel risk assessment form must be returned to the practice 8 weeks prior to your travel date. Please note, not all vaccinations are free and some will incur a cost.
